Everything you need to prepare to come to IMG Academy and stay informed to support your child's journey.

ATTENTION! Incomplete Forms Will Prevent You from Completing Check-In Upon Arrival

This is a critical reminder that your student-athlete will not be permitted to complete check-in when you arrive at campus and begin programming (inlcuding sport) unless all required documents are accurately completed and submitted in advance.

To avoid any disruptions when you arrive, please ensure the following items have been submitted to SchoolDoc:

  • Physician Report – Must be dated on or after 6/1/2025, using the official IMG Academy template with provider signature and stamp.
  • Additional Medical Clearances – Required only if flagged in the Allergies, Health or Orthopedic History or Physician's Report.
  • Medication Authorization Form – Required for prescribed and over-the-counter medications. You will have to print the form, fill it out, and upload it for any medication.
  • Florida High School Athletic Association (FHSAA) EL2 Form (if applicable) – Must be completed by a licensed U.S. physician.
    • This is required for those that play high school Baseball, Basketball, Football, Lacrosse, Softball, Track & Field, and Volleyball.
    • Middle school and post-grad student-athletes will not receive this form to fill out.
  • Current Immunization Records
  • Valid Health Insurance Information
  • Day Students Only – Ensure DH680 compliance

Other things to note:

  • Please allow (2) standard business days to review your documents once you submit all of them.
  • Submission of your documents does not guarantee approval.
  • If this is the case, you will be notified with what needs to be corrected.
  • If everything is accurate, your forms will be approved.

Important Prescription and Medication Reminders

  • A current list of all medications, PRESCRIBED OR OVER-THE-COUNTER, must be submitted in the medication authorization form in SchoolDoc.
  • The medical authorization form must be signed by the prescriber.
  • Upon arrival, all prescription and over-the-counter medications should be provided to Health Services in original packaging with original labeling.
  • All labels must be in English.
  • Full details about prescription and medications can be found in HERE in our Parent Resource Guide.

More detail regarding your required forms can be found in pages 10-19 of the Boarding School Prep Guide.

Additional Information

  • International Resources
  • Housing Portal

The Housing Portal is where boarding students will fill out their Housing Application for the upcoming academic year. The Housing Application will allow you to confirm the demographic information we have on file, allow each student to answer several profile questions pertaining to their living and roommate preferences and give you the option to select a specific roommate to live with for the upcoming school year.

Students must be enrolled to access the Housing Application within the Housing Portal.

Housing applications are now available. ONLY STUDENTS ARE ABLE TO LOGIN TO THE PORTAL VIA THEIR OKTA ACCOUNT.

    • Okta is a single-sign-on app to make it easy for students to login to their various required accounts (Microsoft, Teamworks, Blackbaud, etc...) including the housing portal.
    • This eliminates the need to remember multiple passwords.
    • Once their Okta account is set up, they simply need to visit the Okta Portal to access the housing portal and apply for their roommate matches.
    • Reminder - Okta is only for students at this time.

To set up your child's Okta account, please follow these instructions.

Once their Okta account is set up, they can access the housing portal via the Okta Portal.

If you need help setting up, never got the email from Okta, or the email invite went to your email instead of your child's contact email, please email: [email protected]
